No. MedicalBillingForms.com is an independent educational resource. For official forms (CMS-1500, UB-04, Medicare appeals, state Medicaid), see the Official Forms directory for links to the issuing agencies.
Can I print CMS-1500 or UB-04 forms from this site?
No. The official CMS-1500 (NUCC) and UB-04 (NUBC) forms are not provided here. Many payers require the original red-ink CMS-1500 form for paper claims. Use the Official Forms directory to find sources.
Can these templates guarantee claim payment or appeal approval?
No template can guarantee payment or a favorable appeal outcome. Payer decisions depend on coverage, medical necessity, documentation, and current policies.
